I'd love to have the ability to create forms for students, etc. to apply a certain scholarships or request maintenance for a dorm room...

Carlos Marin We have other web-based forms that are currently handled through our website. The data then flows to a separate database, then we manually re-enter the data into Populi. FORMS would be great! Basically, the same idea of what is available for applicants should be available for current students in a "Forms" tab.

Ben Coumerilh We use forms all the time as well and use an awesome company called wufoo.com. They have an API and it would be awesome to see sone integration with them. :) -
Daniel Humphreys Form creation would be an excellent addition to Populi. This would be especially helpful for course surveys. Just curious, what other uses do you guys have for online forms?

Here's an example of a form that we use for students to register their car. We created the form through our website content manager "Concrete5". When the student submits the form, the student services office which is responsible for issuing parking permits gets an automated e-mail with the pertinent information. This starts the work flow for them to go to Populi and re-key 4 items in the student's info tab:
1. Lic. Plate # and State
2. Most recent date the car was registered on campus
3. Parking Permit # assigned
4. Parking (resident or commuting). I also added a fee that gets automatically generated depeding on what kind of parking permit the student required. So if the Custom Field is: "Parking Resident" then the fee is automatically added to their account every semester (based on the tuition schedule).

Daniel Humphreys So basically, it would be the ability to create a form and link the fields to custom info fields in user profiles, as in your case student vehicle information.
It would also be neat if we could connect the forms to a user's profile so that a link would show up in their alert section saying there was a form they needed to fill out.

Carlos Marin @ Daniel - Correct! The functionality is there kind-of there already (as demonstrated by the Applications Module). Other uses could be creating surveys and standardized testing that is not necessarily linked to a student's academic record.

Christal Fehr We are currently using jotform.com... which is super great and very easy to use (in case you're still curious, Daniel)... but we, too, would love to see that integrated into Populi. I know course evaluations are coming, and we're super excited about that, but there are so many other forms... mostly registration type forms... that we use that it would be great to have within Populi.

Christal Fehr As a step in the direction of total integration, what about having a forms tab where we could embed our own forms that we already have set up elsewhere? That would already go a long way for us.
I love Daniel's suggestion about integrating forms to users so that alerts can show up. We always having trouble getting forms from students, and if there were alerts on their front page, where they could simply click on the form right there, that would be fantastic. Functionality could be the same as what is already used in the "One time Mailing List" section... as far as applying forms to certain groups of people.
I agree that total integration would be fantastic (assuming the forms would be as flexible as what I'm used to in jotforms), but perhaps this baby step could be taken sometime soon? Them's my thoughts :).

Scott Roy I can't really add anything new, but I would love to add onto the requests already here for a form that integrates into current student (or adds new ones) that are flexible to various needs (such as an inquiry into financial aid).

Matthew Pillman Our web manager has a lot of background in database management. It would save a school a lot of time if she was able to access the populi database to upload student info that we collect on multiple other forms!
Otherwise, we should at least be able to create usable forms through populi!

Jonathon Beeke Customized scholarship applications would be our biggest use here. But we would love this also for Independent Study Requests, graduation application forms, extension requests, etc., etc.

Melissa Wilcox We would like this too! We survey students outside of their specific course ie: fall survey that asks about the enrollment process, financial aid process, etc....we don't want to put it in admissions so it would be great to have some sort of forms within Populi to do this. Thanks!

Just like you created a transcript request function with an embedded form for the website, we need various requests to be offered online for our students.
We have student letter request forms, excused absence forms, meeting request forms, schedule modification forms, ...
Please add this vital feature to your already excellent CMS.

I'm all for incorporating forms (intent to graduation; medical information, FERPA wavier; etc.) into Populi.
