Student Request Form

Many times, students request items from the school, which includes official transcripts, tuition refunds, receipts, etc. We have a paper form that allows us to have an official request from the student and this enables us to track who has requested which item. This process also ensures that the requests are being completed in a timely manner. It seems like a good idea to have an electronic "form" that gets tracked through students' profiles which funnels to the appropriate roles to be taken care of. Thanks!
