First off - thanks for getting this feature done for us! I'd like to suggest a few additions that would enhance the functionality of this form.
1) Add Full time or Part time status of the student (perhaps in a column). Usually, health insurance companie not only wants to know whether the student is enrolled but also whether he/she is enrolled at least with part time status
2) Add school logo - have the school logo will help make the form look more official. I believe most (if not all) of the current form exports already pull the school logo so hopefully this will not be too difficult to implement
3) Add generic "Registrar signature" line - when we send the enrollment forms out, the registrar signs the form to give it authenticity. I wonder if this is something that other schools would find useful and thus can be added by default. If not, we'll just export it as ODT and add the signature line manually.
