Attach Enrollment Agreement to Course Registration

Our institution's accrediting agency requires the following:

"If the institution enrolls a student into a single course then the enrollment agreement must cover the obligations of both parties for the particular course in which the student is enrolling. If the institution enrolls a student into one or more courses within a term, the enrollment agreement must cover the obligations of both parties for the particular term in which the student is enrolling"

Our institution's enrollment agreement must include the amount charged, the obligations of the student, and the obligations of the institution (refund policy, etc.)

The agreement must be signed by the student, countersigned by the institution, and delivered back to the student.

We would like the completion of this agreement to be attached to course registration. Student signature could be selecting "I agree" along with required language.

Example:

By selecting "I agree," I understand that this is a legally binding contract. Selecting "I agree" acts as my digital signature and certifies that I have read, understood, and agreed to my rights and responsibilities, and that the institution's cancellation and refund policies have been clearly explained to me. Contract becomes valid and binding when signed by the student and accepted by the institution.

Upon submission, signed contract (signed by student and countersigned by institution) would be delivered to the student via email.
