Course evaluations: Requiring answers specific to previous answer

The subject probably didn't make sense, but I do hope to explain it better.

Let's say I have a Yes/No type of question:
Did you do your homework?

If the student answers "Yes," then he moves on to the next question. If the student answers "No," then a follow up short/long answer question pops up: "If no, then why?"

What we've done in our previous course evaluation is to create the "If yes then why?" statement following a Yes/No question and make it so that "If yes then why?" is also required, even if a student answers no. Many of our students responded to the question asking why they needed to answer it even though they didn't have to (by answering no).

A way to create a "follow up" question to a Yes/No statement will be extremely helpful, I believe.
