Form to be sent to specific faculty member

We have set up a new form, "Request for Class Extension, Incomplete, or Withdrawal." I'd like for a notification to go the relevant faculty member who the request is being made to, as well as to myself (admissions admin). Is this possible? Like, maybe in the field "Faculty Name," could the student choose a faculty member from a drop-down menu, then a notification would automatically go to that faculty member? Thank you!
