Require to input Date of Birth on "Request Transcript"

I would like to request for making Date of Birth "required" when student or alumni requests transcript.

It helps to identify the correct user when multiple users have same first name, last name, and "Most Recent Year Attended.

  Randolph Cheng - We changed it so they are required to enter, in addition to their First and Last names, a SSN/SIN or their Date of Birth.
