We would love to have the option to customize the Transcript Request form. A couple additions we are looking for is an option for being a current or former student and have that link to payment logic (our current students are not charged for transcripts, but former students are). Also, we'd like to customize the SSN so that it only asks for the last 4 of the social. Thanks!

Louis Rakoczy Yes, we would also like to see this as we may require additional information from students. Just as you can have logic within the forms features, we would like similar features as we build a transcript request form.

Brittany Moffitt I agree that a customizable transcript request form would be helpful. I see several other feature requests that might be covered under this topic. For us, we want to add language to the form that would help us verify the requester's identity. For example, adding a phrase like this before the electronic signature: "My signature below indicates my consent for release of my personal academic record to the persons or institutions named above as well as my consent to charge the Transcript Request Fee(s) to the card indicated in the Payment Method information above."
Right now, anyone could log into the public form and request a student's records if the person knew enough of the student's information, but we would not actually have record of the student's consent of the release of these records. This is a FERPA and confidentiality issue. In our previous form program, we were able to monitor this, but the current Populi transcript form does not allow for this functionality.
