Add the ability to send notifications from a form response conditionally, based on responses in the form.

Ie: If requesting program change from program A to B, notify Person 1, but if requesting program change from program B to A, notify Person 2.

Additionally, adding the ability to send notifications to someone else, specified within the form would be great.

ie: when requesting an Incomplete for a course, specify who the professor of the course is and what their email address is, and then we can route the email notification to the email address specified in the form

Yes, please. Especially for email notifications it sure would be nice to use some {!field_name!} kind of placeholders to be able to arbitrarily refer to a response provided. A student wants to make a change and their advisor, instructor, and the registrar all need to sign off ... sure would be nice if the student could choose the adviser from one list, the instructor from another, and the notification would go to all three folks.
