Is it possible to add 2 things to the transcript request form?
#1. If the student requests an email to a 3rd party, may we please have a required field for the student's current mailing address for sending the original physical copy?
#2. Can we please have an "Internal Notes" section for notes for changes or holds. i.e. the student sends a different email address, we can't note it on the form that it's been changed.
