The new Forms feature is great! A suggestion to make it even better:
Add a few more helpful fields in Forms creation: phone number and physical address. The short answer can be used for these but it would be more tidy if it was already in a ( ) _ _ _ - _ _ _ _ type format--indicating area code and digits needed.
A similar field type can be created for a physical address.
